IPD or courtesy runner in 1st inning

Wondering what the rule is if the IPD or courtesy is needed in the 1st inning and there is none out,who goes in to run.

Andy,

For the courtesy runner, it can be any player of the same sex.

For the IPD, if there are no outs then it will be the last batter (same sex) not on base. If there is no last batter not on base, then it will be the last batter (same sex) in the line up.
